A consultation form exists to check fit, not to gather a full training history. Budget, timeline, and goals belong here. Injury history and food preferences belong later, once someone has actually become a client.
A short set of qualifying questions filters out people who were never a realistic fit for your pricing or approach, before either of you spends thirty minutes finding that out live on a call.
Reviewing the answers before a consultation call lets you walk in already knowing what to address, rather than spending the first ten minutes asking questions the form already answered.
